How can I prepare for my appointment?

When you arrive at the office of Dr. James Spalenka, you will be greeted warmly at the front desk, and asked to complete a medical and dental history questionnaire. 


You will need: 


  • To report serious illness in your life. 
  • To report serious hospitalizations, your age and the reason. 
  • To report all major heart, lung or metabolic disease and the age of onset
  • To list all current medications you are taking. 



Please bring personal identification, proof of address and any insurance coverage information you have. 


Accepting All Major Insurance

Insurance Assistance

We will file for you with your insurance company, and will work with you and your insurance company to get the maximum insurance benefit available to you. We accept all insurances except for HMO plans.


Should you have any changes in insurance, please inform us before your appointment.

Frequently Asked Questions

  • What can I expect at my appointment?

    After you have finished new patient onboarding paperwork, you will meet one of our dental assistants who will review your history and clarify your concerns.

     

    The assistant will take x-rays and give them to the doctor to be reviewed. 


    Then you will meet with the doctor for a full clinical exam. 


    Once your concerns have been addressed the doctor will communicate a treatment plan for your consideration. 


    The reception team will make future appointments for treatment and assist with payment options.


  • How should I prepare for my appointments?

    Prepare your medical and dental history or an update. 


    Bring a list of current medications. 

    Write down any concerns or questions you have.


  • Do you offer same day appointments?

    Not guaranteed however, we will accommodate a same day appointment whenever possible.
